Community Health Worker Programs Improve Glycemic Control in Underserved Populations

Abstract

Community health worker programs provide culturally tailored education and navigation support for adults with diabetes in underserved neighborhoods. In a stepped-wedge trial, HbA1c decreased by 0.7 percentage points at six months and diabetes distress scores improved. Program fidelity, supervision, and integration with primary care were critical for sustained outcomes. Policymakers should consider stable financing to scale such models.
