Abstract

Once-weekly basal insulin formulations may simplify therapy and improve adherence compared with once-daily options. In a multicenter observational registry spanning five countries, initiation of a once-weekly basal insulin was associated with a 1.0 percentage point reduction in HbA1c at six months and a modest decrease in total daily insulin dose. Rates of symptomatic hypoglycemia were low and comparable to prior regimens. Treatment satisfaction improved across all age groups. Implementation should include structured titration algorithms and education on missed-dose management.
