Abstract

Sodium–glucose cotransporter-2 inhibitors reduce cardiovascular and renal events in type 2 diabetes. Using a health system perspective over a 10-year horizon, we found SGLT2 therapy to be cost-effective for primary prevention in high-risk adults without established disease, driven by avoided hospitalizations and dialysis. Budget impact is sensitive to drug pricing and uptake. Policymakers should negotiate pricing and prioritize patients most likely to benefit.
